Caring for Veterans

A vast majority of Veterans are not enrolled in the VA healthcare system and may not be aware of the services and benefits available to them, including aid and attendance special benefit, nursing home benefit and hospice care benefit. We are committed to honoring and serving our nation’s Veterans and provide services wherever the veteran calls home, including community nursing homes, private residences and other community facilities.

Our team of nurses, social workers, hospice aides, chaplains, bereavement counselors and volunteers work together to ensure that we honor our Veteran’s unique needs through the utilization of these benefits and our participation in the We Honor Veterans program.

Our social workers are available to assist veterans in discovering the VA benefits available to help with their care needs.

Why “We Honor Veterans”?

  • Approximately 680,000 Veterans die in the US every year; 25% of all deaths in our nation
  • A vast majority of Veterans are not enrolled with the VA healthcare system and may not be aware of the end-of-life services and benefits available to them, including the hospice benefit and VA aid and attendance special monthly pension (SMP).

As a member of the We Honor Veterans program, we are committed to providing excellent care that honor the Veteran’s preferences.

  • To focus on the needs, desires, and treatments of our Veterans
  • To engage Veteran responsibility and accountability in their plan of care
  • To provide collaborative care with the VA and other healthcare providers serving Veterans

How Frontier Hospice meets Veterans’ Unique Needs

We identify hospice patients who are Veterans and determine the Veteran’s needs in the following areas:

  • Clinical Services
  • VA Benefits
  • Bereavement and Grief Support
  • Veteran-to-Veteran Volunteer Support
  • On-going education to our staff, families, caregivers and facilities about Veterans’ issues at end of life and what might influence their needs
